If you would like to receive a free government cell phone from Safelink Wireless, you must first complete the Safelink Wireless application.  The application should take most applicants no more than ten minutes to complete.  To begin the application you must first log onto the Safelink Wireless website and enter your zip code.  If Safelink offers service in your area you will be brought to a page with your state’s basic eligibility information and a link for your state’s Safelink Wireless application.

Currently, Safelink Wireless offers free cell phones in Alabama, Arkansas, Arizona, Connecticut, DC, Delaware, Florida, Georgia, Illinois, Kentucky, Louisiana, Maine, Maryland, Massachusetts, Michigan, Mississippi, Missouri, Nevada, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Puerto Rico, Rhode Island, South Carolina, Tennessee, Texas, Utah, Virginia, Washington, West Virginia and Wisconsin.

By clicking the link you will be brought to a page that provides you with basic instructions on completing the application and the option of choosing a Safelink Wireless application form for applicants claiming program-based or income-based criteria for eligibility (not all states offer income-based eligibility criteria.  If your state is one of the states that only offers program-based eligibility criteria for Safelink Wireless service, you will only see the program-based option).  Select either program-based or income-based criteria (depending on how you qualify for Safelink Wireless service) and press the “Download Application” button.  Once pressed, your Safelink Wireless application should begin downloading.

Once the application is downloaded, you can either print it out and complete the application by hand or complete the application on your computer, then print, and sign it.

The Safelink Wireless application consists of three different sections.  Below is a brief description of each section.

Section One

In Section One of the application you are required to fill out your full name, contact phone number, email, street address, the last four digits of your social security number, and your date of birth.  After this portion of Section One is complete, you will be asked to select which Safelink Wireless cell phone plan you wish to sign up for.  In most states you have the option of selection 68 free minutes a month (with international long distance,  rollover minutes and cheaper texting), 125 free minutes a month (with rollover minutes, but more expensive texting), and finally 250 free monthly minutes (with no rollover minutes).

Section Two

Section Two of the Safelink Wireless application is the eligibility section.  If you downloaded the program-based eligibility application, you will be asked to select which government assistance program you are currently a participating in.  In most states, no supporting documentation is required, but in some states you may be asked to provide written documentation of your participation in the selected government assistance program.

If you selected the income-based eligibility application, you will be required to select what your annual household income is.  Almost all states require written documentation of your household income.  Acceptable forms of documentation may vary from state to state, but they generally include 4 months consecutive pay stubs, letter from your employer, last year’s income tax form, etc

Section Three

Section Three of the Safelink Wireless application is the oath section.  You must sign the application under penalty of perjury that all of your statements are true.  This is a very important section.  So, please review this section carefully and sign the application where indicated.

Once the application is complete, we advice you to double check the application for errors and then return it together with any supporting documentation to the address provided in the application or by FAX to the number indicated within the Safelink Wireless application.
